Please return all documents to the school office as soon as possible.
The Admissions Office must have these items in order to continue with the admissions process. -
- Application for Admission
- Application Fee
- Copy of Birth Certificate with the state seal
- Immunization Record
- Baptism Certificate (for Catholic applicants only)
- First Communion Certificate (if applicable, for Catholic applicants only)
- Recent IEP or 504 Accommodation Plan (if applicable)
- Psychological and/or Psychoeducational Evaluation within last two years (if applicable)
- Most recent Report Card or Progress Report
- Standardized Test Results and previous year standardized test scores (if applicable)
- Child Custody Documents (if applicable)
- Two (2) Teacher Recommendation Letters (if transferring mid-year)

If the student is a Metro Catholic Schools transfer - a meeting must be held with both schools and the family prior to acceptance at St. Pius X Elementary.
